(This form must be completed for all campers under the age of 18 attending camp without a parent)

50th Anniversary T-Shirt Purchase Form
The deadline to purchase anniversary T-shirts was May 8th. Additional spare shirts may be available for purchase at camp.

Reminder: You are not registered until your application form, liability form, and payment has been received.
